My insurance covered at 70 % for mma because Riley and Powell were "out of network" PPO. I was fortunate enought at the time that I had co-insurance through my wife's work and they paid a portion of the remaining 30 % that was not covered.

More than likely it's an out of network situation and you will experience many roadblocks under a HMO system. If your not already on a PPO then get on one and take charge of your health care and the direction you wish it to go.
