They stick a scope in to get a look at your throat while you're asleep, I believe through the nose. That tells them what has to be 'sculpted' to deal with the OSA.

Sometimes somnoplasty is used to reshape the base of the tongue, other times they surgically move the tongue forward so it doesn't flop back in your throat. That's not a really pleasant surgery from what I understand.
