MMA vs Oral splint - help !!!

Hi
I have an RDI of 64 (360 hypopneas and 63 apneas in one night) and used CPAP without success for 2 years.I have used a mouth piece to move my lower jaw forward for the last 6 weeks (made by a dentist with a lot of experience of apnea patients) and have had some success (feel more rested when I wake , less cloudy headed , although still sometimes).

My lower jaw has been advanced by 5 mm - my question is this - how much does the MMA move the jaws forward by typically and are there any reasons why an MMA would be better than a splint ?

Also I worry about my appearence changing if I had the MMA - what have peoples experiences been ?
