There are two procedures for pulling the tongue forward. One is performed by using sutures under the tongue. Not the recommended or preferred method. The other is where a rectangular section of bone is removed from in front of the chin beneath the front teeth. The tendon that support the middle and back of the tongue are then pulled and shortened, the bone section turned 90 degrees and held in place with a titanium screw. I had the second method last April. This along with the hyoid suspension and the somno tongue have reduce my RDI from almost 90 to 40. I was slightly numb for about 10 weeks. It is not as painful as it sounds, I only used 8 percosets during the full week after the surgery and went back to work after a week. For more info try sleepsurgery.com or apneasurgery.com.
