If you get any hassle from the insurance company, start sending letters or emails all the way up through customer service, their Chief Medical Officer, president and CEO if necessary. Explain that the extra time was required for "adequate medical management of the underlying condition as they relate to your situation". I did, also mentioning that it was necessary for my longevity. This action got the insurance company to increase what they are going to pay for my upcoming MMA from $2000 to over $27k and the doctor cannot balance bill me.
