I've heard great things about Kaiser in Oakland for their orthognathic surgery acumen. I don't know if they also specialize in surgery for apnea as that concern is secondary to a skilled bone cutter. Arch bars instead of braces work just fine. The only other concern I can think of is that the anethesialogist be aware of the possible complications. I hear they also make you attend a class two weeks prior to surgery to go over the whole procedure. I've yet to hear any complaints from anyone who went to Kaiser Oakland and apparantly they draw Kaiser patients from all over. good luck/ grumpy
