I guess in anatomical terms, the lower airway lives below the pharynx. Anything above that is "upper". But that aside, the MMA/GA addresses the same area as the UPPP...the posterior oropharynx.
Eventhough, I am kinda down on the lack of good scientific evidence that supports the UPPP, and the fact that it tends to fail more quickly than expected...I also kinda think that the cummulative effect of doing the UPPP+the MMA/GA+ nasal work all add up to an improved airway. I'm not at all sure that my MMA/GA would've been successful if I hadn't tried the others first. And actually, despite the literature and my best instincts, I electively had a UPPP trying to avoid the MMA (which I thought--and correctly so--that an MMA would be a bigger deal).
I go along with the combination findings about the MMA/GA which tell us that they are superior when done together. Therefore, I also tend to support the air-column line of thought that multiple procedures done in phases may be required to open up the entire length of the column.
