I guess it depends on how you define "serious"...
First, there are some annoying possible consequences that could happen even with an ideal or successful UP3. Nasal regurgitation generally involves having a "soda-fizz" sensation in your nose with liquids. Solid foods typically aren't an issue. A foreign body sensation , like a wad of sputum stuck back there, can happen as things scar down. There are slight modifications of technique that can lessen this annoyance.
For some people, the most annoying thing can be that their CPAP mask doesn't seal and they have to switch masks or abandom the device. For me (like you) I couldn't keep it on anyway...so it was a non-issue.
Finally, the worst outcome in my mind is that it represents time, pain, money, and effort for not much gain in the majority of people. While it may contribute to an overall success later when multiple procedures are done afterwards, the UP3 having a low rate of success is enough of a downside by itself.
As for serious, permanent issues after a UPPP...I think you're reasonably safe.
