Trust your surgeon. I wasn't sure if anyone was looking at the whole picture, but obviously they are staging things in a planned manner.

Be prepared to appeal to the insurance company. It seems like standard operating procedure for more and more companies these days.

At some point, if it looks like insurance will NOT budge and you don't want to rescedule, you might ask your surgeon if he offers a "cash" package discount. You might have to take a short term loan as this will generally cash up front. If you have to pay for it out of pocket, it could save you money.
