how to pursue MMA

I don't know if I should go to Stanford to get a consult from the surgeons there or not. This would be a large out-of-pocket expense and wonder if it would make more sense to have the surgery with a good surgeon in my area (Phoenix).

Does anyone have good sites or ways to research doctors on this surgery in different cities? I want to go to a reputable doctor that has some experience with MMA for sleep apnea.

I almost don't want to deal with insurance (currently I have United Healthcare). For those of you who got your surgery covered easily, did you have to present proof that you were on CPAP and other previous treatments?
