There are only a couple of surgeons who perform the MMA just for sleep apnea. (Atlanta and Stanford).
I had mine done in Boston by a guy who does maybe ten a year for sleep apnea. Lots for other reasons.
I had an easier time of getting it approved in part because my surgeon was in network. Also, because I live in Boston, a very good city to get sick in.
Many of these posts about insurance are upsetting to read...in part because people feel betrayed by their insurance company(rightly so).
But if you go in with the attitude that won't cover, but try hard to get them to anyway, you may have an easier time of it.
Yes, you should have proof you tried and failed Cpap. What is proof? Nice long letter from your sleep doctor. Then, nice long letter from your surgeon, Then, Nice long letter from your primary care physician. Sure, through some affidavits in from relatives who have watched you suffer, all your sleep tests, etc.
Get the pre cert.
As for Phoenix, there must be ONE top oral surgeon there. It is a big city.
