I am marshaling my insurence guns. I had my sleep doctor write a letter stating that I had exercised all the non-invasive options and that CPAP is not going to be effective for me. He was really supportive of moving forward. The surgeon is writing for authorization too (probably already submitted).

This surgeon does not accept insurence, he is cash up front only. I already have his fees which are pretty reasonable compared to some cited here. It's about $17-18K for jaw and nose in two seperate procedures. The price is probably lower than some others noted here because he has his own JAACO(?) approved surgical suite, so I save $$ over paying for the hospital OR. I'll be staying the first night at a Hyatt with a surgical fellow in the next suite.

Thanks again for the tips. I am pretty nervous about the surgery, it's the first time I've been operated on. However, I'm determined to proceed since there is a pretty good chance that addressing the OSA will improve my quality of life.
