You should not do it. Your sleep doctor should write the letter. You are the patient. Doctor is MUCH better.

"Patient X first came to my office in June 2000. Sleep studies showed a level of apnea high enough to interfere with sleep. Shows classic signs of sleep apnea. Interferes with quality of life (XYZ sucks because of Apnea).

Patient tried Cpap, but could not comply with mask. Patient could not compluy with Bi-pap. He had a septoplasty to comply. Tried seven kinds of masks, and a three full face masks. Tried a dental device. Still, could not comply.
Patient made a good faith effort to comply with Cpap protocols. Removes mask unconsciously during the night.

Only option is surgery, otherwise doom and gloom.

Siincerely,

Doctor Doctor

You need something like this from a bunch of people at least you Your sleep doctor and your surgeon. I had to pay my surgeon 1,000 for a full workup. He wrote an exhaustive letter to the insurance company, and it got approved.

For petes sake, don't say the claustrophobia thing. That opens the door to, "Try Harder." You wake up with it off. Let the doctors sell you.

Honestly, it hurt to pay the 1000 dollars(not covered by insurance) but it got results. I never had to write a letter or anything like that.

Good luck.

mike

p.s. this is just my take on things. There are plenty of ways to do things, and I could be wrong.
