You're going to hear this a lot:

No insurance company will approve expensive surgery such as orthognathic procedures if you haven't gone through (and failed) a course of CPAP treatment. While your self-diagnosis may turn out to be correct, there is no short-circuiting the system.

Try CPAP. You're going to have to give it 6 months at least. You must have your doctor/sleep specialist document if and why it does not work. Then you can start making the rounds of surgeons and ENTs and building your case.

I can't stress this enough - you need to get on CPAP stat. OSA can do cumulative damage to your health. Get some treatment and then (with a clearer mind and more energy) look at your options.
