I'd rather not dig up all that paperwork from a year ago. Besides, it varies so much with the Dr and Hospitals. The price your Dr and hosptial would charge is no secret, just ask. I will say this, my private State Farm insurace paid nicely and promptly - I'm very happy with them as usual. But, I would have hocked the house if I had to.
