First is you ENT a sleep specialist or at least treats OSA patients with trachs. If not I would suggest you find one that does.
Breathing with a trach does take some adjusting. You don't say what size trach. It could be it is not the right size but only your ENT can tell you this for sure. It might not be the right kind of tube for you. There are many different type of trach tubes--again discuss the different ones with your ENT. If you don't know the different types of tubes let me know and I can give you some links. Are you lightheaded during the day with the trach capped? or at night uncapped? or lightheaded when you are in bed on your back?
I assume the reasoning behind the "never go without the trach being capped" is for you try to get use to breathing normally while capped.
I never cap my trach because when I do my pulse rate goes up about 30 points.
While there has been great discussion on "the best, or correct or whatever" trach tubes on this forum. Keep in mind that what is right for one person is not necessarily right for another.
If I can be of further assistance please do not hestitate to let me know.
