Try taking your pulse just resting and uncapped. Then resting and capped. Then moving around and capped. That might give you some indication if you heart is working too hard when capped.
Most OSA people do cap during the day. Especially if there are no other medical conditions.Most people I know that keep an open trach have either other medical conditions and/or are obese.
Can you breathe through your nose or is it blocked?What does your ENT say is causing you to be light headed?
I talk normally with my trach when I finger occlude. I have never had a bad reaction to my trach. I also do not cover my trach--it simply is more comfortable for me. However you can wear a scarf just don't let it go over the trach if you have it uncapped. With practice there are ways to have an open trach that most people will never know you are finger occluding. For instance often when I am talking to people for the first time I will put my hand along my neck & ear and use my thumb to occlude. This looks like I am resting my head on my hand. Movement of my thumb is very little only enough to control the air flow.
However if you have no other medical conditions affecting your breathing I strongly suggest you keep trying to cap off a little at a time then 15 minutes more each day until you build your time up. It can't be the size of the tube that is the problem since it is fine when sleeping. Talk to your ENT--tell him the problems you are having and what can you do about them?
