Lisa, don't give up. Sounds to me like you may very well need a larger tube. I have been trached almost five yeaars now, and like you I had a very hard time with the size 6. I have been using the Jackson size 8 for years now. I don't have to have a tube with a cuff because I do not need to bi-pap, so I really can't help you with that. There is a guy that post to this forum that uses either bi-pap or vent at night and does not use a cuff tube. I will try to find his email address for you. Hang in there it really can get better, most of the time I never even think about my trach and I keep it open 24/7 and am very active. I also never cover my trach or try to hide it at all, and yes I get a few looks but given the chance I tell people about how it saved my life.I also finger occlude it just works out better for me. You can however, use a speaking valve, you may want to ask about one. I'm not a doctor and only know what works for me but ask your doctor about two things that will give you a lot more air. One if you can use a tube without a cuff. I do know that people do use bi-pap and vent without it. Two the most important for a larger tube! Good luck!
