I AGREE!!! You need to see someone who performs this procedure on a very regular basis. They may have some insights from other "problem" cases. I have heard that some patients have had the rear molars removed and the jaw section during the forward movement to prevent a drastic protruding jaw.

And Yes, cpap doen't work for everyone. It took over two years and a lot of research on my parts to help determine that cpap helped to induce centrals at about the same rate that it stopped the obstructives. You just need to be diligent and seek other opinions and fixate on one that you feel comfortable with.
