Of all the questions you pose, this is the easiest to discuss...
The Pillar is nearly useless in moderate to severe OSA. And, may even be marginal in mild OSA. The data that led to FDA approval was for safety and did not show great efficacy.
The combination of the two may yeild positive results in some people...however, that should be carefully examined by sleep study. While the oral dental devices provide help with some people and are definitely a boon to people intolerant of CPAP, again, the cure rate...and consistent use rate is very low!
If you have tried the dental device and tolerate it...you may want to try the pillar as well. However, anatomically it doesn't trully add to your airway. It just takes the reed out of the woodwind instrument and prevents on-going vibration of a floppy palate. If you don't tolerate a dental device...then a pillar procedure is definitely a waste of time and money !
