There are some prominent surgeons who get excellent physiologic results...but they do so by maximizing their advancement. Literally saying "If you need the surgery, you'll get my best advancement effort. If you don't want that, you don't need my help."

Now that is old school bravado and a bit extreme. However, there is an element of truth to what they say. You can't tie someone's hands too much...otherwise you'll go through alot with an MMA and may have an incomplete result. A lose-lose situation. And, what these guys say is true...you must accept risks, or be willing to face changes to get results.

If you aren't there yet...then perhaps you should try everything else and completely rethink surgery entirely. All of us were there...mentally and/or physiologically at the end our rope!

Fundamentally: NOTHING IS MEDICINE IS A FREE LUNCH! Meaning, every drug you take, every procedure you undergo...has risks and side-effects. You must never assume or expect anything to be free from risks or side-effects! Hopefully, they are minor, or you are in the percentage that doesn't experience them. But, when you take any medicine, herb or procedure...you must be prepared as best you can.

My surgeon at Stanford has a "rolling" technique for the mandible to minimize cosmetic effects (but not eliminate them!) and minimize the effects of forward advancement. However, no one can inhibit change brought on by maxilla advancement. It will flare your nostrils, etc..as the most common cosmetic side effect (this can be mitigated by simple rhinoplasty later). Although I am convinced that he is great at achieving good cosmetic results...there will be changes in facial appearance that only you ...and you alone can decide whether or not it will be worthwhile.
