Any good orthognathic surgeon will have techniques to minimize negative facial changes with MMA.
That said, maybe your apnea is not very severe. Otherwise, doing something to greatly reduce or cure your apnea would have a higher priority and facial changes would have a lower priority than they appear to now.

There are a few people who have posted about negative changes, as this topic (cosmetic change) is a sensitive one. One specifically said she would gladly do it again, *even though there was a negative change that had to be addressed with subsequent cosmetic surgery.*
And, if you look at the history of this forum and others, people are NOT afraid to post about bad results, problems etc.
However, most people have either said it made no difference or IMPROVED their looks.
You do face a risk, especially since you had jaw advancement before. You need to decide whether the chance of improved health is worth that risk, and then manage that risk by finding a top-notch surgeon who is willing to balance your health vs. appearance to your satisfaction.
