In my case, it (releasing the hyoid suspension) turned out to be appropriate. My hyoid area "popped" one day while I was shaving and sleep became less common after that. We suspected that one of the related poly sutures may have loosened or possibly even have failed. Dr. B. wanted the possibility to start from a clean slate in regards to the hyoid area. They had a palate/throat specialist assist with making the decision (actually a whole team of folks was in cahoots on it). If your team is not comfortable making such decisions before, and yet reserving the right to possibly change them during, surgery; then you may wish to look around. My Dr.s said from the start that they would adjust and look for resulting changes to determine if the adjustments were appropriate and effective. In fact, they made two sets of splints. One set moved me forward 7mm and the other, which they finally used, moved me 10mm. They wanted to get sufficient airway opening with minimal impact. They all got together and determined that 10mm didn't make my face look "bad" (low on my personal priority list) and that it helped open the airway sufficiently with minimal risk to palate and throat closure needs such as swallowing, talking, etc. (high on my personal priority list). So, 10mm advancement and release the hyoid it was!
By the way: I'm up now because the weight of my ice bag pressing on my throat slowly makes it hard to breathe and I wake up. But - it's temporary, in fact; swelling will go down from here and I'll soon be sleeping (dare I say? - on my back?) with no ice. And I sleep for as much as 1.5 to 2 hours until that wakes me up. Yes, I have been getting vivid dreams. I don't mind the recovery period's minor difficulties. It's still so much better than getting a few preciuos yet desparate minutes sleep while inevitably cutting off airflow altogether!
Did I mention that all of this darned bed rest has my back a little sore? ;>)
