As a UPPP is most often not a cure...YOU MUST GET A REPEAT SLEEP STUDY! And, this is best done after at least 3 months...
No visual tests while sleeping. An MRI is quite noisey and a specific device that is a giant magnet that will not work with leads and other devices. The only things to mimic airways during rest and sleeping are a fiber-optic exam with the Mueller maneuver...so see how your tongue collapses. The vast majority of people have a significant tongue-base obstructive component...that is why UPPPs don't really work in a majority of people (it never addresses the tongue base).
It seems as though you need a much more thorough sleep evaluation. One that will define the areas of obstruction better...and a better sleep test, with perhaps a split night test that evaluates CPAP pressure settings that will work best. Additionally...there are full face masks that work very well for some people...meaning your mouth is covered too ...
The other thing that will improve your breathing and CPAP use is diminishing the size of those turbs! But, that's another story. That is a very easy procedure to tolerate...much better than the UPPP by far!
