I have UHC and have had this procedure.

This procedure needs to be pilled as a genioglossus advancement -- is what a GA is. If authorization is being requested as a sliding genioplasty, it will not be approved because they are correct, it is a cosmetic procedure.

I actually had a modified sliding genioplasty but it was authorized as a genioglossus advancement because it was "modified" as used for the treatment of sleep apnea.

A new authorization needs to be called in by your doctor's office for genioglossus advancement. With some luck they may get the approval in time. If not, you may have to push your surgery out of a few weeks...unless you're okay with paying OOP.
