Thanks for the responses.

My surgeon has send an appeal letter today. I will follow up with UHC to make sure it was received (amazing how they don't receive things) and then push to get it all in writing.

I guess I will have to mentally prepare to not have to seriously consider not having the procedures if I can't get everything in order.
