How have you shown you NEED surgery

I've been using CPAP successfully for a few years but would really like to ditch the CPAP forever. How do you prove that you NEED surgery or do you not have to? It seems like the insurance companies could easily say to just use the CPAP, it works. If you want an unecessary surgery, pay for it yourself. Thanks
