Not sure how you document want vs. need. In my case, 2 plus years of no noticeable improvement, the development of claustrophobia and noticeable increase in central apnea while on cpap helped. These plus a letter writing campaign letting the insurance company know they were messing with my longevity and to let them know that surgery (MMA) was "the best possible solution for the medical management of the underlying cause for obstructive sleep apnea" turned them to my side.
