Well, I was a believer in the maxim "Never ascribe to malice that which is adequately explained by incompetence" (Napoleon Bonaparte).

However, when you see incompetence repeated in systematic ways, or at widespread levels, you have to wonder.

I don't know how it is in your state, but Washington State has a very strong insurance commissioner's office. You may need to get them involved. In Washington at least, they have been VERY active in going after insurance companies successfully.
