My surgery was postponed twice due to last minute partial refusal of the insurance company. I ended up with letters from the doctor stating necessity as to the surgery being the best "treatment for the medical management of the underlying condition". That along with my letter stating they were cutting short my chance at longevity, they finally granted the procedure and the doctor ended up waiving my out of pocket because of the work I did to get it approved.
