jay,
Just wondering if there's a step-by-step game plan or advice Bill and Lisa gave you for fighting this. I'm guessing you 1st contact your insurance company to see who to deal with? Then get your doctor to send them a letter explaining the necessity of the procedure? I have a Bluecross PPO and usually they've been pretty good at covering the costs of the surgery, at least regarding hospital surgical expenses which is usually the most expensive part is usually covered by my insurance (i.e. the hospitals are usually in-network with bluecross). But I often deal with surgeons/doctors who are out-of-network with bluecross, and so I end up paying an arm and a leg for the doctor's fees later on.
