Wow. You'd do another MMA? My MMA also didn't help cure my apnea as well. Hindsight is 20/20, but I probably should have gone to Stanford (or to this other doc I originally consulted with) even though they're both out of network. But even if they suggested I go in for another MMA, I probably wouldn't. The MMA isn't a 100% guarantee that your apnea would be cured. Not to mention is the 3 months of discomfort (pretty severe the first 6 weeks) that I don't ever want to go thru again. If it were me, I'd try to get a sleep endoscopy so the doc can see exactly what my problem is during sleep. In my case, the doc found out that I had Laryngeal obstructions that required a partial epiglotidectomy. After getting this simple surgery done, in my case it improved things considerably more than what the MMA did. Very ironic.
In any case, even supposing that the results of my sleep endoscopy says that I'm a candidate for another more aggressive MMA, I'm not sure if I'd want to go thru that ordeal again. I'd probably try the trach first. For all practical purposes, the trach gives me a 100% cure from obstructions since it bypasses the upper airway where obstructions occur. It's also much less invasive a surgery than an MMA, and it's also reversible. If I have a trach done and I still have severe sleeping problems, then I'd know my problem wasn't just limited to OSA. I'd then try to seek proper treatment for whatever it was besides the OSA. If, on the other hand, I'm sleeping like a baby with no more OSA, then I'd know my problem was strictly OSA. I can then decide to either live with the trach (which does take some lifestyle changes and adjustment), or decide later on to go in for that MMA or some other surgery that the doc might recommend.
