How common is it to have only the lower jaw extended?

I just had another sleep study and I'm back up to 30 times an hour. The doc is wanting me to try to make CPAP work again which I'm going to try. The CPAP got the events down to 5 per hour.

I use a dental appliance now. It had reduced events to 15 times an hour previously but evidently my body has adapted again.

I'm concerned that if I switch to CPAP my jaw will recede again because the appliance will no longer be stretching my jaw forward each night.

That's got me thinking that I should consider having my lower jaw moved forward permanently. Not using the appliance should have the added benefit of making more room in my mouth for my tongue. It seems like my mouth is crowded and even more so when I use the appliance.

Any thoughts or experience with this?
