Someone has already mentioned scarfs, turtle neck shirts, wide neck chokers (lots of models wear them). This is where creativity comes in and I can tell you of a lawyer who went to Stanford in the late 70's, before CPAP, had a trach and is still using it today and practicing law. Some people really like them over cpap or surgery.

You can do a site search for the Hole in the Throat Gang and you will find lots of information about working with them.

I did find this online, which using a stainless steel band.
