As Bill said, pain can raise blood pressure. I've always had BP of < 130 all my life. This spring, I was in unexpected chronic pain from gum surgery for 4 solid weeks. I went in a few weeks back for a checkup and my BP was at 145!

I was taking therapeutic doses of ibuprofen which can also (over 4 weeks) raise BP.

You are right to be alert, but don't stress about it.

You didn't say how long you are post-op. Do you still have pain? And you mentioned a traumatic ICU stay - memories of that could certainly increase stress and raise BP.

I would consider a little counseling/therapy. It might help you to handle the day to day as you recover.
