It's VERY subjective. The more talking and customer contact, or more physical, or more mentally demanding, the longer recovery time until you can cope.
I planned on two weeks, with the third week working from home. My job entailed a lot of difficult analysis, report writing, and people contact. Going back after three weeks was SUPER hard for me. I don't think I was truly on top of my game for closer to eight weeks (as learian mentioned).
If I did it again, I would have probably taken medical leave (without pay) for a month in addition to two weeks of vacation. Then going back after 6 weeks would have been a bit tough, but not too bad.
Obviously, we all do what we have to, subject to financial and other considerations. Four weeks might be OK for you, but don't count on being 100% in the 5th week...
