Yes, it is very common for surgeons to have two prices - the insurance price, which assumes they may have to wait months to be fully paid (I think it was 5 months in my case); and the cash price, which is always significantly lower but is 100% paid before surgery. In my case it represented a 35% discount (I had insurance so I didn't take that path).

Some may do a payment plan - it's worth asking.

Bear in mind that the hospital charges can equal or exceed the surgeon fee! I think in general it might be harder to work out creative financing with a hospital. Again, always worth asking, you never know what you might find out...
