From googling, 21145 looks like a LeFort I osteotomy. That is the upper jaw advancement.

The number of codes you submit for preauthorization depends on how many codes your surgeon is going to bill the insurance company for. If your surgeon is only billing for 21145 & 21196 then that's all you need to worry about.

You might also have to get preauth for the hospital stay. Depends on your insurance company's policies.
