Sounds like you already have a pretty good body of data to present to the ins. co. regarding the effectiveness, or lack of, of CPAP. I'm not sure who lives one street away from you, unless it is in Palo Alto, but you might check out the "Ask a Sleep Surgeon" page on this site. You won't find, in my opinion, a better dr. to diagnose the various problems you're dealing with. However, I would think any ENT who has had advanced OSA-related training would be able to help you.
