This sounded like the soft palate was closing until you said pushing your jaw forward helped. I don't know how pushing your lower jaw forward would prevent the soft palate from sealing off the upper airway unless it hangs way down. I doubt it's the epiglottis because it's bent the other way. And I doubt it's the vocal folds, but not impossible.

If, while the seal is happening, can you open your lips and release the air? If so, it's gotta be above your tongue - soft palate or turbinates. If not then below - epiglottis, tongue, vocal folds.

Definitely tell your Dr about this. A sleep endoscopy could probably tell, or maybe your Dr can tell with the fiberscope in his office.
