Struggling with CPAP, What's next?

Hello,

I appreciate all the info I found here.

I was diagnosed 4 years ago. I've had a UPPP, nasal turbinate reduction, deviated septum fixed, and somnoplasty on the base of the tongue. All about 3 years ago.

I'm still using CPAP and a dental appliance, but its not resolving things very well. At a pressure of 13, the AHI is still usually above 10, and I'm exhausted. Sometimes the AHI is lower and I feel better. There's definitely a strong correlation between what machine thinks the AHI is and how I feel. And the pressure of 13 seems to as be good as it gets. Things are worse either higher or lower.

The AHI isn't all that high, but I feel awful.

I have no compliance issues with the CPAP. I wear it all night every night. Its just not getting the job done. (I'd still love to ditch it, but living with it would be acceptable if it was resolving my apnea). And I've pretty well exhausted all the options is making it work better (using the humidifier, full-face mask, leaks aren't an issue).

I'm moderately overweight (5'9", 185 lbs). But not all that much. I'm in fairly good shape, so my BMI is really lower then my height/weight would suggest. But still probably 15 lbs heavier then I should be. Point being my apnea is probably not attributable to my weight.

So I'm looking for new options. I'm thinking further options are:

Further RF work on the tongue.
A GA
A MMA

I'll be see the folks at upenn in a few weeks.

Other thoughts on options? Anybody have experience with these options and upenn?

I've been adjusting the idea of MMA for a while now. About now I'm happy to put up with the recovery if that's what it takes.
