I have seen only one sleep endoscopy. Trying to sleep with an endoscope down your nose is very difficult. Other factors are obstructions typically occur in more than one place. So with the scope in place and viewing an obstruction in the upper part of the throat you would miss an obstruction at the base of the tongue.
Another factor, that prevents it from being a good tool for evaluating obstructive locations during sleep, would be the presence of the device itself during sleep causing an abnormal air flow.
Using it during wake doesn't really show what happens during sleep so when laying down on your back you can't know for sure the exact location of the obstruction.
As medical science advances, we will soon develop a non-invasive, practical, not expensive, safe, and accurate way of observing obstruction locations during sleep.
Even then, if you spot just one obstruction and have surgery to correct it, the removal of that obstruction can create the environment needed for an obstruction to happen at another location.
That is why MMA's are more successful than other surgeries. They pretty much treat the entire airway, where most of the other procedures treat specific sites.
