I would give it a few nights to see how it goes. Apnea can be
variable at night. Sometimes patterns need time to play out. I
would expect apnea to be worse on your back as you mention.
The surgical options are aimed at reducing the size and
floppiness of the palate and reducing the size of the tongue or
moving it forward in some way. The MMA works by bringing the
palate and tongue forward as well as increasing the tension of
the muscles of your mouth.

Again- most surgical procedures that are "soft tissue" are
frequently temporary. There are a number of surgical
procedures that one can have done.

1)Palatal pillars (usually worthless)
2)somnoplasty of tongue and palate
3)Repose system of the tongue and now the hyoid bode
4)Hyoid advancement
5)Genioglossal advancement
6)uvolopalatoplasty (UP3) which has a number of modifications
that can be done on the original concept.

7)There is a relatively new procedure to reduce the tongue size.
It is referred to by some as:
Submucosal Minimally Invasive Lingual Excision (SMILE). It is a
variation on the radiofrequency technique (somnoplasty) used
on the tongue and palate. It was originally used on children for
adenoid and tonsillectomy. A plasma wand is inserted into the
base of the tongue and when activated, liquifies the muscle of
the tongue. The advantages over radiofrequency are that it can
be done in fewer sessions and in areas not accessible by
somnoplasty. However, it is a new technique and studies
looking at efficacy and safety are lacking. It seems like it could
be a good thing but it we need more clinical studies and
experience.
