I think you have to weigh the pros and the cons. There are no free lunches. For sure, there will be undesirable side effects. Make no mistake about that. On the other hand, sleep apnea is killing you. It is killing you slowly but surely. In my opinion, you have no choice other than to take Phase two surgery. WIth an AHI of fifty one, how long before you get diabetes? How long before your hearg gives out? Night after night, your body is taking a pounding.
Abotu the Phase one surgery, I am not surprised that you didn't get any better. Usually, people don't do well. Phase two tends to produce more significant positive results.
I advise you to do two things: Get phase two, and make your peace with CPAP if it is at all possible. If you cannot do those things, you will have to consider a tracheostomy (breathing hole in the neck). Sorry to be blunt, but I am a realist, and I am telling you as I really see your options.
