In my case, phase 1 was the pharyngoplasty with a tonsillectomy - not an UPPP. Dr. KL has a fantastic write up on how he does the procedure -http://www.sleepapneasurgery.com/pharyngoplasty.html.

While I'm sure there are many people who visit Stanford and advance directly to an MMA (Dr. KL would know best, as he likely sees many of those patients), my bet would be that the people who go to phase II probably have a higher AHI/RDI than I have and/or have shown some therapeutic improvement with xPAP or other conventional treatment.
