I had a similar issue a year ago. My initial RDI was 21.1 and my RDI post MMA was 21.8 that said, I feel better post surgery than before even if this is not perfect. IMO, one f the reason why my RDI was higher post surgery is because I had more allergy for this second test but I cannot be sure, as I realized recently that I was allergic as I lived with them forever, like I did with apnea...

So in the last year I focused on allergies and potentially depression. If the allergies are real, depression is more questionable... I have already stop most of the med as it did not do much...

I am using APAP with a 10 to 20 pressure and in the morning it is between 10.5 and 12 and report AHI of 2.5 to 5.0 with 0 to 0.5 AI. Note that, I am coming back from 9 days of vacations where plug-in an APAP was not an option and I did not feel so different but I was on vacations, so way different condition.

So for the next step, I am planning on going back to my surgeon and discuss the options, including possibly the new option described by Pinsky above: http://www.sleepnet.com/noncpap25/messages/212.html
