Hi Biam,

I read your previous post which stated:

So for the next step, I am planning on going back to my surgeon and discuss the options, including possibly the new option described by Pinsky above: http://www.sleepnet.com/noncpap25/messages/212.html

I don't know if you remember me I posted in the "Same RDI after MMA surgery" thread. We have the same doctor who I hope 1) does not read this post and 2) does not know who we are.

Could you please inform the board of his next surgical suggestions? I spoke with him regarding my post-MMA sleep study result where the RDI was the same as pre-surgery but my sleep has improved significantly but is definitely not at 100% He said that even though my RDI is the same the airway is improved and the degree/amplitude of hypopnea/flow limitation are higher. He said my brain is very sensitive to respiratory events.

I tried cpap which is working MUCH better but do not get the formal titration until 9/12. Before the MMA surgery, cpap was not working at the max pressure of 20 cm. Pre-surgery, I said that I wouldn't mind wearing cpap as long as it worked. But now, since I had a great recovery and I feel so close to a cure. Perhaps this is where my reasoning is flawed - perhaps OSA can only be "treated". Yet I can't help but wonder how helpful additional surgeries may be i.e. I guess this is human nature. I am curious about genioglossus advancment, hyoid advancement surgery but I don't want to seem like a surgery addict. Also the surgeon said that these surgeries are unpredictable in terms of surgical success. He vaguely mentioned that he is currently working on new treatment options. He also mentioned that I don't have to wear cpap forever, just for the time being which is true.

I guess I'm obsessing over sleep apnea too much but when you've finally had a taste of "normal" afters years of suffering you want to do everything in your power to hold onto that feeling.
