I highly recommend getting the auto bipap – I use the Respironics PR System One. I had the mma surgery for mild sleep apnea (RDI 14). I also had many flow limitations. Even though my apnea was mild, my symptoms were very debilitating. My mma surgery resulted in a 50% improvement of sleep quality and has made cpap therapy more effective as it did not help pre-surgery. After the mma surgery, I was retested and still had sleep apnea, RDI of 14 although I felt improved. The first post surgery titration resulted in a recommended pressure setting of 11 cm. I set my auto cpap to this pressure setting but due to my different jaw structure, I was swallowing air (aerophagia) and air would automatically escape through my mouth. It would dry my mouth out every hour even with the full face mask. I bought a bipap off of Craigslist and noticed that my sleep was MUCH improved and I did not have aerophagia or mouth air leakage. My pressure needs change throughout the night and I was not titrated on the bipap so I was not getting optimal sleep although I felt better. I went back to my sleep doc at Stanford who prescribed the auto-bipap. I am about 90% better on this machine. They did not titrate me on it though based the pressure settings on my cpap titration. I increased the pressure setting myself a couple of points and felt better. By the way, I have also been following Dr. Krakow and found a lot of his points to be valid i.e. auto-bipap is the way to go even if you pressure is less than 15 cm and that sometimes your optimal pressure settings is more than your titrated setting.
