Years ago, I found the right surgeon for my MMA redo. And BCBS would not re-imburse at normal levels, so surgeon has to have his fee up front. Hospital charges are supposedly covered. And NO-ONE at Blu Cross/Double Cross..has ever heard of the gap exclusion. The lady at surg office is good, and she said best thing is to get re-imbursement amount from ins company in writing...problem is, they wont do that. Good luck. In my case, i dont have the fortitude if both things go wrong. That is, if my surgery is botched again, AND i lose the 50k in braces and surgery. So they have beat me down enough to give up.
