Your plan is a good one, and worked well for me (100% coverage eventually paid by insurance.)
My insurance company referred me to several local doctors who the insurance company claimed performed sleep apnea surgery. I was able to get each of those local doctors to refer me to the sleep surgeon who ultimtely performed the MMA. My MMA was done at Stanford by the surgeon of my choice and all of the doctors originally suggested by my local insurance company practiced within 15 miles of Stanford.
